Summary of Responsibilities:
The Director of Operations will be a part of our national team of operations leaders managing a specified region and participate in the development, planning, and operations for a multi-physician practice to fulfill programmatic directions and goals and oversees the operational integration of various and new programs into Skilled Nursing Facilities (SNFs) and other healthcare settings.

Provider Relations and Performance: The Director travels to conduct provider introductions and interfaces directly with our clinical providers ensuring smooth on-boarding credentialing, training, and implementation into Skilled Nursing Facilities and other healthcare settings. 
Facility Relations and Performance: The Director of Operations manages each individual account to ensure success for all parties as well as collaborating with the client building&rsquo;s administrative teams to ensure consistent, efficient, and compliant operational performance.
This is a remote role working from home with frequent travel/driving locally to oversee operations, meet facility leaders, and conduct provider onboarding and training. 
The Director of Operations will report to the SVP of Operations and collaborate with the SVP of Business Development for growth initiatives.

Education and Skill Requirement:

Must have 2 years of leadership and administration experience in the Skilled Nursing Facility, Nursing Home or Hospital Rehabilitation healthcare setting (in Admissions, Operations, BD/Marketing, Social Work, Therapy).
Bachelor&rsquo;s degree in related healthcare fields.
Cannot be excluded from participation in any state or federal programs such as Medicare or Medicaid.
Proficiency with HIPAA and PHI compliant practices. 
Proficient in healthcare management software (EMR, HMR) and Microsoft Office Suite (Excel, Word, Outlook)
Practice excellent verbal and written communication skills in remote settings and present in a professional manner in both in-person and online environments.
Must be a self-starter who is disciplined, motivated, detail-oriented, organized, and able to prioritize tasks in a fast-paced environment.

Position Summary:
 
The Hospital Coordinator serves as the primary operational liaison between hospital-based providers, hospital support staff, post-acute partners, and administrative teams. This role is responsible for coordinating daily provider operations, managing patient tracking and assignments, supporting transitions of care, facilitating communication across the continuum, and ensuring accurate documentation, billing, payroll submission, and scheduling activities.
 
Essential Duties and Responsibilities:
 
Provider Operations & Scheduling

Coordinate and maintain provider coverage schedules.
Schedule provider meetings, operational reviews, and leadership meetings.
Coordinate C-suite meetings, Medical Operations Reviews (MORs), and other administrative meetings.
Serve as a liaison between hospital support staff, providers, and operational leadership.
Support provider onboarding and ongoing operational needs.

Patient Assignment & Tracking

Track hospitalized patients and assign providers based on payer relationships, PCP affiliations, and organizational guidelines.
Monitor patient census and maintain accurate patient assignment records.
Verify that all assigned patients have been seen by the appropriate provider.
Follow up on outstanding patient encounters and rounding activities.
Track patient discharge orders and anticipated discharge dates.
Monitor patient movement throughout the hospital stay to ensure continuity of care.

Care Coordination & FastPAC Support

Facilitate patient handoff to the FastPAC program and post-acute care teams.
Coordinate communication between hospital providers, skilled nursing facilities, PCPs, care managers, and other stakeholders.
Support transition planning activities and ensure appropriate information is communicated during care transitions.
Schedule PCP follow-up appointments upon discharge when appropriate.
Assist with care coordination initiatives designed to improve patient outcomes and reduce readmissions.

Clinical Operations Support

Attend interdisciplinary rounds two to three times per week.
Track action items and follow-up needs identified during rounds.
Communicate patient status updates and discharge planning information to appropriate stakeholders.
Assist providers with operational workflow management and issue resolution.

Documentation, Billing & Payroll Support

Verify completion of provider documentation and encounter requirements.
Support billing workflow by ensuring documentation is complete and submitted timely.
Assist with payroll submission processes and provider productivity tracking.
Maintain accurate operational reports and performance metrics.
Support audits and compliance initiatives as needed.

Requirements:


Associate's degree; bachelor's degree in healthcare administration or over 8 years of successful performance and meeting below requirements.


2&ndash;5 years of healthcare operations/project coordination and organization, care coordination, physician practice management, hospital administration, and proficiency in managing multiple priorities in a fast-paced healthcare environment with strong attention to detail and follow-through.


At least 2 years of experience working within hospital, post-acute, or physician practice environments with knowledge of healthcare workflows, patient transitions, and provider operations with proven track record of building successful relationships with physicians, hospital staff, SNF partners, and community providers.

The Post Acute Transition Coordinator plays a critical role in ensuring seamless patient transitions across the continuum of care&mdash;from hospital discharge to skilled nursing facility (SNF) admission and ultimately back to the community. This role is responsible for tracking patient movement, confirming timely provider engagement, facilitating communication between care teams (including primary care providers), and ensuring appropriate follow-up care is scheduled post-discharge. The Transition Coordinator is a key driver of care continuity, patient outcomes, and reduction in avoidable readmissions.
Key Responsibilities
1. Patient Transition Tracking

Monitor and track patients discharged from hospitals to SNFs within assigned regions
Maintain accurate, real-time tracking logs of patient status across care settings
Ensure all patients are assigned to Medrina providers upon SNF admission

2. Provider Engagement & Visit Confirmation

Confirm that patients are seen by Medrina internal medicine and/or specialty providers within required timeframes (e.g., within 24&ndash;48 hours of SNF admission)
Escalate gaps in coverage or delays in provider visits to operations leadership
Track completion of initial and follow-up visits

3. Communication Coordination

Send structured communication to:

SNF partners upon patient arrival
Internal providers regarding new admissions
Primary Care Providers (PCPs) at key transition points


Ensure PCPs are:

Notified of hospital admission and SNF placement
Updated during SNF stay as appropriate
Informed of discharge plans



4. Discharge Planning & Follow-Up Scheduling

Coordinate with SNF staff and Medrina providers to identify anticipated discharge dates
Schedule follow-up appointments with PCPs and/or specialists prior to patient discharge home
Confirm appointments are documented and communicated to all stakeholders

5. Documentation & Reporting

Maintain accurate documentation of all outreach, communications, and scheduling activities
Track key metrics including:

Time to first provider visit
PCP communication completion rates
Follow-up appointment scheduling rates


Generate reports for operational and quality performance monitoring

6. Cross-Functional Collaboration

Work closely with:

Regional Operations Teams
Providers (MDs, APPs)
SNF administration and case management teams
Care management / CCM teams

Support broader initiatives such as:

Readmission reduction programs

Education & Experience Requirements:

Partner with VP of HR to continuously improve recruiting and talent management workflows for efficiency and scalability. 

Compliance and Inclusion: 

Ensure hiring and selection processes are compliant with EEOC, Federal and State laws.
Facilitate presentations and training to advise hiring managers/interview teams on recruitment and interview practices.
Support HR audits and reporting related to recruitment and staffing data.
Foster a culture of inclusion in hiring, selection and retention practices. 

Job Requirements: 

Bachelor's degree in HR, communications, psychology, marketing, or a related field; or 10 years of experience in recruitment and selection with SHRM-PHR/CP certification.
Minimum of 5 years of experience in talent acquisition and retention initiatives with strong interviewing and relationship building skills.
Proficiency and experience with ATS and HRIS platforms and MS Suite (i.e. Outlook, SharePoint, Excel, Word, PowerPoint) and Adobe.  Proven ability to learn various software quickly.
Proven experience in developing candidate pipelines, implementing talent acquisition initiatives, and retention strategies.
Strong analytical and data-driven decision-making skills.
Excellent communication and interpersonal skills in-person and in remote settings.
Ability to manage multiple priorities and work effectively in a fast-paced environment in a remote workplace setting, and strong initiative to assist and be an excellent team player.
Attend weekly and bi-weekly virtual group meetings via MS Teams, as well as weekly calls with VP of HR and hiring managers. Join us for team watercoolers and training sessions. 

Physical Requirements:

This is a full-time remote, work-from-home position, primarily working during standard business hours handling sensitive and confidential information. Occasional changes in schedule are required to accommodate candidate interviews in the evenings.
Work must be performed in a private and secure setting requiring reliable internet and phone connectivity.
Ability to communicate via virtual meetings daily with a camera on as well as being responsive in a timely manner during work hours via email, MS Teams and phone.
Ability to occasionally travel for company training, meetings and recruiting events.
Ability to sit, stand, and walk for extended periods, and lift and move up to 25 pounds.

Your mission in this role will be to streamline the patient&rsquo;s experience, allowing our physicians to focus on delivering exceptional treatment without any hiccups. What You&rsquo;ll Do:

Phone reception duties.

Scheduling appointments and assisting with questions relating to appointments, medications, authorizations, and other care related inquiries
Creating telephone encounters within EHR system, relaying messages with name, call back number, and detailed reason for the call if physician or managerial attention is required.


Assist with patient preparation prior to visit.

Performing reminder calls to patients at appointment times.
Verifying patient insurance eligibility utilizing medical group/IPA portals and insurance portals
Verifying patients have prior authorizations for patient visits, if required
Updating insurance, prior authorization, medical records information


Monitoring faxes

Responding and forwarding faxes to responsible parties
Uploading authorizations or patient documents into EHR


Maintaining inventory and clinic supplies

Ensure work site has sufficient amounts of inventory at all times including medical supplies, kitchen supplies, any other supplies utilized by office.
Collect inventory and supplies requests biweekly.
Receiving and verifying incoming package and contents


Scanning, faxing, emailing duties.

Scanning and uploading patient documents into EHR
Faxing or emailing documents to patients, hospitals, skilled nursing facilities, pharmacies, other doctor offices
Fax or Email documents when requested.


Provide front office support.

Greeting patients, checking patients in, checking patients out
Collecting and scanning patient co-payments, insurance ID, government ID
Assisting patients with new patient packets, arranging transportation, consent forms


All other duties as assigned.


What You&rsquo;ll Need:

GED or High school diploma required.
At least 1 years of experience in reception area in healthcare setting.
At least 1 year of EMR Experience (experience with Practice Fusion EMR is preferred).
This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines
Intermediate proficient computer Skills &ndash; Use of Microsoft Word, Excel, PowerPoint, basic office equipment.
Strong verbal and written communication skills.
Knowledge of Medical terminology.
Work is performed in an office environment.
Must have the ability to push, pull, lift and or carry up to 25 pounds.
